Case: A 75-year-old man presented to the ER with fever, SOB (shortness of breath), chest pain, and severe, extremely productive cough. The patient had been a heavy smoker for almost 50 years before quitting 7 years prior when diagnosed with emphysema. The patient occasionally used O2 at home when he had difficulty breathing and was using oxygen at the time he was admitted to the hospital. A chest X-ray showed a right lower lobe infiltrate diagnostic of pneumonia. Sputum, urine, and blood cultures were collected.The gram stain report of the sputum specimen is as follows:
many neutrophils were seen (> 25 per low power field)rare squamous epith cells (< 1 per low power field)many gram pos lancet-shaped diplococci (> 25 per oil immersion field)few gram neg diplococci and gram pos bacilli (< 10 per oil immersion field)
