The nurse is standing at the charting station on a surgical floor of an acute care teaching hospital. As the nurse reviews the client’s medical record, the surgeon approaches the nurse and states: “I am in a rush, and I need that client’s chart right now!” The surgeon sticks out his hand for the nurse to hand the medical record over. The nurse feels flustered and angry because the nurse is not finished with documenting assessment findings.
