At some point, you have probably considered the health problems and issues in your own community or a community with which you are familiar, because the health of those you care for is of importance to you. Consider how reassuring it would be to know that public health officials were conducting an effective community health assessment prior to targeting health efforts.

For this week's Discussion, review the guidelines provided for the Scholar-Practitioner Project, located in the Resources. Select a real community, either the one in which you live or another one that interests you, on which you will focus during the course for your Scholar-Practitioner Project. Then, select a public health problem for this Discussion.

With these thoughts in mind:

Post 3-4 pages- a description of a community that you chose to assess in this course for your Scholar-Practitioner Project. Include in your description sociodemographic and geographical characteristics (e.g., urban, rural, race, ethnicity, gender, or age distribution). Describe what you perceive to be a public health problem in that community and explain why. Describe two contributing factors relating to the public health problem you identified and explain how they contribute to the public health problem. Then, explain two potential solutions to the problem.
