CC: Chest Pain HPI: The pain began 45 minutes ago while watching television and is described as severe and pressure-like. It is substernal in location, radiates to the jaw and left shoulder, and is associated with shortness of breath. Nothing makes the pain worse or better. Medical Hx: HTN, HLD, Right Knee OA Surgical Hx: Right total knee replacement 2018 Social Hx: Current Smoker 1PPD. No ETOH. No illicit drugs Family Hx: Mother died at 80 yo from Breast CA Father died at 55 of MI Immunizations: Up to Date Allergies: Ciprofloxacin: RASH Medications: Simvastatin 5 mg oral daily Amlodipine 5 mg. ROS: General: Positive Diaphoresis. Negative fever, chills, weight loss HEENT: Negative for sore throat, congestions, ear pain Neuro: Negative for headache, seizure, dizziness CV: Positive Chest pain. Negative for palpitations, orthopnea, edema Pulm: Negative shortness of breath, cough, wheezing GI: Postive Nausea, negative for abdominal pain, diarrhea, constipation Urinary: Negative for dysuria, hematuria, flank pain Heme: Negative for bleeding, easy brusing Skin: Negative for rash Exam: General: Patient awake, alert and in moderate distress Neck: Supple, no JVD CV: RRR s1 s2 no murmurs, rubs or gallops Pulm: Breathing unlabored, CTAB anteriorly Abdominal Soft, NT, ND, +BS Extremities:
