INTRODUCTION
In this assessment, you will conduct a community needs assessment that will outline the needs of a population and demonstrate how a risk-bearing model will meet these needs. The needs assessment will also justify how an organization will financially support and sustain the transition to population health management by implementing a new service line. You will assess potential payment models and systematically determine a viable strategic recommendation.
The information and analyses from the three course assignments should be used to complete this task.
Note: Each of the course assignments, including the "Pro Forma Operating Budget," should be completed in full before attempting this task. The three course assignments are submitted as Task 1.
SCENARIO
You are the administrator of Corazon y Alma Health, which includes both a hospital that is currently a member of an Accountable Care Organization (ACO) and includes the Corazon y Alma Medical Clinic. Corazon y Alma Health serves the population of Miami-Dade County, Florida. In this role, you are accountable for creating a strategic plan that will provide optimal access to higher quality, more cost-effective care, delivered in the most appropriate clinical setting. You are also accountable for recommending a value-based model that will provide accessible, cost-effective, patient-centric care to a targeted high-risk patient population. This will require an examination of financial models, volume projections, payers, regulatory requirements, barriers to care, and stakeholder interests. You will be making your recommendation of this value-based payment model and strategic plan to the board of directors of Corazon y Alma Health.
REQUIREMENTS

Create a strategic plan for the proposed prenatal service line for the board of directors of Corazon y Alma Health by doing the following:
Population Health Management
A. Write a summary of the "Population Risk Assessment Worksheet" from Task 1: Course Assignments that includes the following:
• the target population
• the significant health determinants
• barriers to care for the target patient population
• a justification for pursuing the prenatal service line
B. Write a recommendation for a value-based payment model that Corazon y Alma Health should adopt to support implementation of the proposed prenatal service line.
Financial Feasibility
C. Conduct a financial feasibility analysis for the proposed service line by doing the following:
Question 1. Provide the "Pro Forma Operating Budget" for the proposed prenatal service line at Corazon y Alma Health that you completed in Task 1: Course Assignments.
Question 2. Discuss how variance analysis could be beneficial in mitigating any risks associated with the rollout of the new service line at Corazon y Alma. Use financial data from the pro forma worksheet in support of your discussion.
Question 3. Explain how the projected operating budget for the proposed prenatal service line at Corazon y Alma Health supports your choice of a value-based model.
Question 4. Discuss potential funding options for the prenatal service line.
Question 5. Discuss the anticipated effects on Corazon y Alma Health by using the Strategy and Finance dimension of Miller's Impact Assessment Framework.
Question 6. Identify two existing regulatory agencies that could significantly impact operations at Corazon y Alma Health.
D. Describe current or new infrastructure requirements to support the prenatal service line in the following areas:
• provider network
• physician champion
• patient access (risk mitigation for barriers to care)
• data management, analysis, and reporting to various audiences (e.g., physicians, community)
E. Discuss three key financial benchmarks that should be included in a balanced scorecard for evaluating the overall effectiveness of the value-based model.
F. Create a change management plan for the proposed prenatal service line by doing the following:
1. Identify the group of leaders (e.g., advisory group or a guiding coalition) that you will invite to provide valuable insights as part of the model development phase.
2. Identify potential internal and external stakeholders.
3. Describe strategies to keep internal and external stakeholders informed and engaged in the change management process.
4. Describe the strategies you will use to empower others within the organization to problem solve and make decisions during the change management process.
5. Describe the strategies that you will use to address barriers to change.
6. Identify the benchmarks for short-term successes within the change management plan.
G. Acknowledge sources, using APA-formatted in-text citations and references, for content that is quoted, paraphrased, or summarized.
H. Demonstrate professional communication in the content and presentation of your submission.
