Case: You will then write a plan to address one priority population health concern in the community. Your plan will include implementation, at least two goals and three objectives, and an evaluation plan. You will also need to identify a theory or model you will use to develop your plan and explain why you chose the specific theory or model.

Your community will be Beaumont, TX. You can determine your boundaries, which determine your population. Since you will not be able to directly speak with your population in most cases, you will need to research news and media to get community input.

There are many sources of information you will use for this assignment. Some great sources of information include city websites, CDC websites, and hospitals' websites as hospitals must complete a community assessment every three years and make that assessment public, news, crime statistics, etc... The sources of information are endless.

This is a formal paper. You do not need to have page headers. You are expected to provide information for each of the Grammar, APA, and spelling counts so make sure you proofread.
