INITIAL QUESTION:

Mary, a baccalaureate prepared registered nurse, has been practicing for fifteen years. Throughout her nursing career, she worked in pediatric, surgical, and emergency departments. She worked as a floor nurse, charge nurse, and was recently offered a position as a nursing supervisor. A stipulation for becoming a nursing supervisor is that Mary must attain a Master of Health Administration degree within 2 years of accepting the position. The offer prompted Mary to contemplate her career. She is interested in returning to college, but she realized that she does not want to pursue working in administration and away from patients. Mary decided to become an Advanced Practice Nurse (APN). Mary knows that working as an APN will allow her the opportunity to make a larger contribution to individuals, environment, health, and nursing. She researched graduate nursing programs and discovered that there are four roles of the APN. Mary must choose one role and apply to a program, but she is unsure about the different roles and their individual scopes of practice.

How would you decide? Develop a pros and cons list to assist Mary in making her decision. Include each role of the APN on the list and be certain to provide appropriate rationales and citations.

1. Nurse Practitioner: The Nurse Practitioners (NP) work either in a hospital setting or a clinic. It relies on the interest of NP
Pros: With becoming an NP, a nurse becomes doctorate or trained by the Masters. It enhances the status of a nurse. NPs can give best care to the patients. In addition, NPs can give recommendations to the patients as regards to their treatment or physician that they should choose for their treatment.

Cons: Initially, a good time is required to be invested to become a NP doctorate. In addition, NP is required to be extreme expert in giving health recommendations to patients because negative outcomes of recommended action may put the patient at risk.

2. Nurse Anesthetist: These nurses provide anesthesia to the patients.

Pros: Nurses learn the skill of collaboration with the whole medical group. In addition, these nurses gain a large amount of medical knowledge, which enhances their status.
Cons: The procedure of giving anesthesia to the patients is a complex procedure. Therefore, these nurses are required to have a thorough knowledge of the human physique such as weight, vital signs, height, etc. Errors are not tolerated in this stream.

3. Nurse Midwife: These nurses have the job of operate directly with the mothers all through the gestation period.

Pros: These nurses hold the capability and knowledge to give advice to mothers. Nurse will have the entire knowledge about the unique health concerns faced through mothers. These nurses also have the privilege to assist the medical groups to make sure that child and mother get the superior care.

Cons: These nurses are required to tackle the unexpected cases that may confuse them in giving treatment. This is the most difficult part of the job of such APNs.

4. Clinical Nurse Specialist: These APNs operate the broader number of patients instead of working with single patients.

Pros: These APNs work as the leaders inside health care organizations. These APNs also have the chance to educate the patients about their health, although their goal is always to provide cure to the patients.

Cons: These APNs distress themselves with essential societal and health related influences. Therefore, these APNS may face huge anxiety (Kindschuh, 2016).

From considering the above roles of APNs, it can be said that Mary will chose the role on the basis of her interest in giving medical care.

Nurse practitioners who are certified in New York are allowed to practice in one or more field of speciality areas, including adult health, Gerontology, Psychiatry, School health, Paediatrics, Women's health, Oncology, Palliative care, Holistic care, Obstetrics and Neonatology. A nurse in New York is allowed to provide nursing services under the laws if he or she is competent to deliver the services (Iglehart et al., 2013). A NY nurse practitioner is not allowed to provide nursing services in areas where she or he is not competent to perform, even if the New York law allows a nurse to offer the services. Only a licensed practitioner is allowed to provide competent care as recommended by the New York laws and if one is not licensed, you will be charged with misconduct. New York laws restrict the use of the title nurse to only those people who are certified by the New York State Education Department, such as certified nurse practitioners, registered nurses, and licensed practical nurses. For one to practice as a nurse in New York, you must be licensed registered by the New York State Educational Department. Nurse practitioners are legally required to wear identification budges indicating their names and the professional title if you are practicing in a hospital, clinic, group practice or other establishments that are offering health services to the public. Business ventures are not legally authorized by the New York laws, and are not allowed to hire licensed nurses to provide nursing services in New York State. A licensed nurse with a private practice is allowed to retain patient records for at least six years. Some records like the one of obstetrics and the records of the minor patients must be retained for at least six years. It is against the law for not maintain the patient records which reflects the accurate evaluation and the treatment which the patient was on (Zaccagnini et al., 2015). The state laws of New York does not allow the licensed physician to delegate tasks which fall exclusive within the practitioner's scope of practice to another licensed professional, such as registered nurse, LNP and CNP. Nurses are allowed to share their concerns with their clinical supervisor or to report any observation to the New York State Department of Health bureau of Controlled substances.
