"Cy reported lower back pain while therapy was with him Pt was reported to be grimacing with movement. Redness was noted to the bottom and reported and pt. verbalized pain to nursing staff. OP reports that Pt has a palmer cushion on his wheelchair which helps with positioning. NP would like 7-day Wand to be applied weekly on shower day Thursday AM shift. Pt wheel chair should be in a reclined position to held with the pain. Pt. reports less pain with reclined wheelchair. Pt has orders for Tylenol 100 mg BID for pain and PRN discontinued."
