Abir Kassem is a 62-year-old woman who attended the emergency department of her local hospital with mild chest pain. Accompanying her was her husband, oldest son and daughter-in-law. Although Mrs Kassem arrived from Lebanon with her husband 20 years ago, her English is hesitant, and her husband answered for her when questioned. However, he would not respond to any questions of a personal nature and the nursing staff found it difficult to take her history. It was only when the resident doctor, Dr Ashyra Singh, arrive to examine Mrs Kassem that she was allowed to be examined by her and her daughter-in-law permitted to speak on Mrs Kassem's behalf. Following the examination, it was decided to admit Mrs Kassem to the cardiac ward for further observation and for treatment of chest pain.
During the morning medical consultation ward round, you note that Mrs Kassem's daughter in-law is not around. When the doctor requested to assess Mrs Kassem, she got very agitated and uncooperative.
