The RN, working in a behavioral health outpatient clinic answering crisis telephone calls, takes a call from a patient's boyfriend who reports that he is concerned about her. The patient, a 28-year-old female with a diagnosis of bipolar disorder, might have attempted suicide by overdosing on her medications. In exploring this further, the RN learns that the patient is exhibiting drowsiness with incoherent speech at times, muscle weakness, and lack of coordination, resulting in her falling as she attempted to rise from the couch to use the bathroom secondary to severe diarrhea and vomiting for the past 4 to 8 hours. Her list of current medications includes Celexa 40 mg daily, lithium carbonate 600 mg twice daily, and a daily multivitamin tablet. Further information gathering reveals that oddly, the patient has not appeared depressed or discussed any thoughts of wanting to hurt herself. In fact, her mood has actually been quite stable, and she was recently promoted at work. She has been making plans for her future; she has been training for an upcoming marathon race despite the recent heat wave with temperatures reaching 104°F during the past 5 days. Despite the heat, the patient has continued to run faithfully each day.
