Position Description : Social Worker- Drug Health Services

Maximise the functional status of the client/patient and carer in the optimum environment and facilitate safe discharge from the service.

Document all aspects of care including education to clients/patients/carers, progress notes and referrals to health care providers.

Communicate appropriately with clients/patients/families/other health care workers at all times.

Provide appropriate service delivery in accordance to client need and within the limitations of the service's resources and in accordance with evidence based practice.

Facilitate and promote client/patient/carer participation and feedback to ensure the service is meeting their needs Attend clinical service and departmental meetings as required

Participate in quality improvement projects that is relevant to the clinical area and evaluate the effectiveness of service provision and impact on client/patient outcomes

Maintain administrative systems/data entry related to the booking, scheduling and counting / recording of clienVpatient/ contact as required for mandatory reporting and as required by manager for planning, quality or research

Maintain professional standards and participate in the professional development program including mandatory training?

Participate in regular formal supervision according to the SLHD Allied Health supervision policy.

Will be required to work at any SLHD site upon request from Line Manager

Selection Criteria

1. Must hold a Bachelor degree in Social Work or equivalent that provides eligibility for full membership of Australian Association of Social Work.

2. Demonstrated knowledge, skills and clinical experience in Social Work practice and the provision of services.

3. Demonstrated high level written and verbal communication skills and excellent interpersonal skills.

4. Demonstrated high level of organizational and time management skills

5. Demonstrated commitment to professional development, evidence based practice and continous improvment.
