2. Analyze, using a financial perspective, the difference in types of costs that affect the organization's operating budget resulting from a decision to deliver care to the target population using a population health, wellness-based model as opposed to the current illness-based model.
D. Apply Wagner's Chronic Care Model to your population health initiative by doing the following:
1. Describe two care strategies for each of the six elements of the Chronic Care Model that can be customized to improve patient care for your target population.
a. Analyze the anticipated impact the strategies from part D1 will have on both the healthcare outcomes and the overall health of the target population.
2. Explain how care coordination would specifically address one critical healthcare risk described in part A1 for your target population.
3. Discuss the benefits of care coordination in supporting health outcomes for any one of the critical healthcare risks described in part A1.
E. Create a sustainability strategy by doing the following:
1. Describe your incentive plan for the successful participation of internal and external stakeholders in the implementation of the population health model.
2. Discuss how financial risk mitigation strategies may influence stakeholder participation in the incentive plan.
3. Discuss the potential positive and negative impacts of your population health model, using the attached "Miller's Impact Assessment Framework."
4. Evaluate your identified organization's ability to support the strategies in your population health initiative based on your responses in this task.
F. Explain how your population health initiative aligns with the Institute for Healthcare Improvement (IHI) Triple Aim model of healthcare.
