For example, you may choose heart disease and identify the elderly population. Identify strategies for improvement regarding this health concern with this identified population.

In addition to using academic sources to help create a health action plan, consult with at least one professional in your community. The person you consult with could be a nurse or someone working within the health care field.

You can do this via phone, in person, or through the Internet.

Think about what questions would be good to ask:

What are some current outreach strategies used with this population?

Are there local community resources that work with this population?

Are there other community resources that might help increase health awareness for this health issue?

What are the biggest challenges for health providers to address this community health issue?

Then, use this information to act as a nurse leader to develop a plan to deal with this issue.

Identify communication technologies that could be utilized to contribute to effective communication.
