Case Study - Urinary Tract Infection (UTI)

You will first create a care map of your case current situation (see care map template and instructions in PPT attached) then you will write a brief paper to explain your reasoning and choices.

You will create a care map for the patient that you are assigned (case studies) patient is at center

Enter the assessment data on the left side (type in the needed data) do not go overboard and try to fit in everything. More is not better.

Put in information about the patient's health situation (factors, internal/external that affect health) in the box.

Identify at least 3 Gordons patterns and include the data that supports you picking that one.

Identify 3 nursing diagnoses and PRIORITIZE them.

How? ABC? Maslow? Be able to support your choice.

Identify interventions for your PRIORITY problem (use your text not your best guess).

Identify what will be in your educational plan (what will be taught to address that educational diagnosis).

Identify learning goals and outcomes (how do you know you were successful).

Draw arrow that show the relationships between your various components.

Now please write a BRIEF paper that explains your care map in words. This will show your thinking. Each Gordon's Patterns box will be a paragraph.

Then answer these in the rest of the paper:

1. What were the identified nursing diagnoses and why? Remember to use the: "Problem" Related to "_____ " Evidenced by "____" format.

2. Which nursing diagnosis is most important to address with this client? Explain how this was determined.

3. Which nursing diagnosis is second most important to address with this client? Explain how this was determined.

4. Which nursing diagnosis is least important to address with this client? Explain how this was determined.

Then Complete the Nursing process by answering the questions below and including the information in the last paragraph.

What is the goal for the client to show that the priority problem is reduced or resolved?

What would a nurse (you) need to do for the client to help the client meet the goal?

How would you know that the client has met the goal?

Now please write a BRIEF paper that explains your care map in words. This will show your thinking. Each Gordon’s Patterns box will be a paragraph. Tell me about all the thinking you did to develop the associated nursing diagnoses for that pattern. Then, move on to the next and make that a new paragraph. Don’t assume I know anything about your thought process, because I don’t. Show me how you are thinking! The draft of this is due next week (week 7) I will communicate my concerns if any for revisions. The final product will be due week 9 (end of class) Good luck!

Note - USE your text books for appropriate nursing and educational ideas. You are not an expert in the field so your ideas (while probably good) are NOT what I want here. I need you to find interventions and plans in your text book. The goal is that you stretch yourself and learn, not just tell me what you would do at home. Some of you have not been citing references on assignments and are getting downgraded already- CITE REFERENCES IN APA FORMAT.
