Construct a PowerPoint that includes a critical thinking narrative of your choosing that outlines a case study in which a gerontology client is considered at risk (due to age, social support, and/or the nature of her or his acute or chronic problem/s or disorder/s). When presenting the case, include any tips on how to submit referrals, cost, pros/cons of the service, barriers to care, points of contact, family support, interdisciplinary collaborations/involvement, and long-term vs. short-term goals.
The goal of this assignment to is utilize your skills as a practitioner in finding meaningful interventions that include resources within your local community. This assignment will require that you highlight resources, programs, support groups, home health, hospice, palliative care, and/or other entities available in your local community to help care for your selected patient.
The PowerPoint presentation will need to include a case study complete with SOAP descriptors (subjective, objective, assessment, and plan). Please, clearly identify the problem and the impact on the patient's overall health and/or quality of life. Within the PowerPoint, describe your community resource(s) and how you as a provider can use the service(s) to help your patient. Be specific in your choice of resources. If you are using examples such as Meals on Wheels, hospice, palliative care, support groups, etc., please ensure to give specific examples of these options in your local community. Students who list the "provider" as the resource will NOT receive credit for the assignment. Students may also address any lack of resources or barriers to care within their communities that may present obstacles for providers and those they serve.
