Question
Frank Geller is a 76-year-old retired teacher. He was hospitalized three weeks ago following a motor vehicle crash. He suffered multiple fractures and lacerations of his liver and spleen in the accident. He was intubated and placed on mechanical ventilation shortly after arrival to the hospital. During the following weeks, he developed ventilator-associated pneumonia. His sputum cultures indicated that the causative organism was methicillin-resistant S. aureus. He is currently being treated with IV vancomycin. Over the past week, Mr. Geller's renal function has deteriorated, with rising blood urea nitrogen and creatinine levels and decreasing urinary output. His physicians are considering hemodialysis as a treatment option. During a physical assessment, you note a white coating on Mr. Geller's mouth and tongue. The physician is notified, and she diagnoses a Candida albicans infection of the mouth and prescribes nystatin oral solution.
1. Would Mr. Geller's pneumonia be considered community acquired or nosocomial? What factors could have decreased his resistance to infection?
2. Discuss the significance of the methicillin-resistant S. aureus infection. What factors have impacted the increasing incidence of antibiotic-resistant microorganisms?
3. What impact will his renal function have on his treatment with vancomycin?
4. What would you identify as the probable reasons for the Candida albicans infection?
