Ms. J is a 77 yo Caucasian female that has lived in a Long Term Care facility for more than 10 years. She has a son and a daughter that lives in a different state. She has a medical history of Parkinson disease, hypertension, and diabetes. She has been taking Carbidopa/Levodopa 25/100 mg tid, Lisinopril 40 mg, and Metformin 500 mg bid that has been controlling symptoms well over the past few years.
While rounding in the facility, the nurse informed me that Ms. J has been having frequent falls due to increased gait instability, cognitive impairment, increased hand tremors while sitting, and weight loss. She has been displaying over the last week visual hallucination and delusions. She will not eat the food because she feels that she is being poisoned by the staff. Nurse also states that the patient has told her that she has been seeing snakes coming out of the wall.
What will be your initial plan of care for this patient? Would you consider stopping or adding medications to her regimen? What will be your primary diagnosis along with the rationale ?
