PB is your patient in room #3 on the medical unit at the county rural hospital. PB is a 70 year old farmer with a medical history of COPD and hypertension. He was admitted yesterday with an exacerbation of COPD and drove himself to the hospital as he couldn't breathe properly. This is PBs 3rd admission to the hospital this year for the same complaint. You take PBs vital signs first thing in the morning - BP 158/90, HR 65, RR 24 and slightly laboured, with O2 sats of 89% on 1L nasal cannula. PB admits to smoking ½-1 pack of cigarettes/day or even more ever since his wife passed away 2 years ago. When asked about coping with the loss of his wife. PB states he is still grieving. His home medications include ramipril 10mg PO daily, Symbicort inh BID and Ventolin 100mcg inh PRN. He states his puffers ran out a few weeks ago and he only takes them when he needs to. The doctor has ordered ABGs and you are waiting on his results.
