Basic concept map for pain.
Patient Scenario:
Patient Jane Doe is a 68-year-old female who presented to the Emergency Department with nausea, RUQ and epigastric pain that began shortly after eating dinner. She describes the pain as 10/10 "the worst pain I've ever experienced in my life", constant, sharp, stabbing, and reports radiation to the R shoulder. After blood work (CBC/CMP), UA, and a CT of the abdomen were completed in the ED she has been admitted to the observation unit for cholecystitis and is scheduled for a cholecystectomy tomorrow morning.
While in the ED J.D. has received multiple doses of morphine 4 mg IV (for pain) and zorfran 4 mg IV (for nausea) without relief. Her current pain level is an 8/10 and her last dose of morphine and Zofran was 20 minutes ago.
Last set of vital signs: BP 166/84, HR 110, SpO2 98% on RA, Temp. 98.9 oral, pain 8/10. Patient appears restless and is guarding her abdomen upon palpation.
PMH: HTN, elevated cholesterol. Both are controlled with medications (Metoprolol 25 mg BID and Simvastatin 40 mg QHS). No previous surgical Hx.
