Scenario:

A 9-month-old boy comes to the clinic with his mother for his well-child visit. His mother specifically wants to address his immunization status and make sure he is up to date. She hands you his state immunization record. You see that he hasn't received his #3 (Hib) or (IPV) immunization yet.

Because his mother is in a hurry to get to her own doctor's appointment, you help expedite Andre's catch-up shots by administration of the shots. After beginning to write in the patient's chart you read a note written 3 months prior when the patient saw one of the other providers. The note states that the patient received the Hib and IPV #3 during that last well visit. It is now evident that the patient's immunization card was not completed during that last visit and the patient has received 2 of the #3 Hib and IPV vaccinations.
