Case:
Mrs. Saburit, a 52-year-old patient was diagnosed with diabetes mellitus and is currently taking Metformin 1gm with breakfast and 500 mg with supper. She had no known complications to her diabetes and she had a history of chronic alcohol abuse when she was young. Mrs. Saburit was admitted to the Medical wing of FUMC -Antipolo because of vomiting, carpal tunnel pain, leg pain, tingling and numbness of the lips, hands and feet with severe body malaise. Upon interview, the patient also reported significant diarrhea which had lasted since she begin taking metformin. When asked about the characteristics of stools, she told the nurse that it was loose and on an average of 4x a day and as if it contains mucus in it. Her additional medications include metoprolol, clopidogrel and atorvastatin.On the physical examination of Mrs. Saburit, her BP was 180/100 mmHg, serum Na of 143 mEq/L, serum K of 2.9 mEq/L, serum Ca of 1.2 mEq/L, serum Mg of 0.4 mg/dL and a fasting blood glucose level of 156 mg/dL. Mrs. Saburit was also positive for Trosseau's sign and Chvostek's sign.Mrs. Saburit's primary care provider ordered parenteral IV magnesium sulfate via infusion pump and included in her diet oral magnesium salts. She was advised to eat foods rich in magnesium.
