Assignment - Geriatric Survey Questions

Description - Complete an assessment with the senior adult (68 years old Female). You should ask the questions and enter their answers. Download the survey answers.

Ask your participant what gives their life meaning (Family and living healthy life), and what they see as areas they could improve on that would improve their Quality of Life (Excersing, eating healthy, keeping up with their doctors visit for regular checkup and maintaining healthy lifestyle). Collaborate with the senior adult to set three short-term and three long-term goals that would bring more quality to their life.

Then collect all the data and write a paper using the following as a guide.

1. Describe your applicant, including information about their age ( 68yrs) gender (female), general health (Hx of Diabetes), and comfort level with the interview.

2. Analyze the QOL survey you performed.

3. Identify three areas of concern that you see involving risk. (Risk of Health complications related to diabetes e.g Neuropathy, Kidney failure, Hypertension)

4. Discuss what components of a successful aging theory applies to them.

5. Discuss how this person's life aligns with qualities of health in the Blue Zones.

6. Collaborate with your participant in creating three short-term and three long-term goals that they feel are most important to improving their quality of life.

7. Describe what you have learned about your role as a nurse practitioner in this setting.

8. Ensure that your paper is no longer than 3 to 5 pages, not including the title and reference pages.

9. Use APA (7th edition) formatting for in-text citations.

10. Use at least three scholarly references, preferably from provider-based or NP-based journals.

11. Use the Quality-of-Life template to organize your paper.
