There are Two Sections in whcih the assessment is meant to answered

Section 1 in 500 words
Section 2 in 2500 words

A total of 3000 Words( kindly check attached for more details about how this sections are meant to be answered)

Learning Outcomes 1. Critically discuss the core principles of health promotion, participation, collaboration, empowermnet and equity.

Learning Outcomes 2. Critically review the skills nurses require to assess health needs and to support people with the identification of their own health needs.

Learning Outcomes 3. Critically appraise behaviour change theories and their use.

Learning Outcomes 4. Demonstrate skills and knowledge required to seek out opportunities which promote support client centred health choice.

Also ceratin books and journals are required to be used and referenced in the assessment report.

You are required to analyse and critically discuss a health promotion opportunity you have been directly involved with or have observed within your student nurse practice.

The work should be presented in 2 sections.

Section 1 (500 words)
This section is a written description of your chosen health promotion opportunity.
• The emphasis of this section should be the health promotion activity and where relevant some background information relating to the client should be included, however client confidentially should always be maintained.
• In this section you need to clearly indicate your role as the health promoter (or the role of or the role of the health promoter) as this will be the main area for discussion in section 2.
• There is no requirement to add references to this section.

Section 2 (2500 words)
In this section the health promotion activity described in section 1 should be analysed.
The health promoter's actions should be considered in relation to the theories and topics discussed in the unit.
• The work should discuss the client's perception of health, relevant definitions of health promotion and the skills and knowledge nurses require to work in an empowering way when promoting the health of the client(s).
Links between theory and your practice should be made throughout the submission. Relevant wider reading will help you to demonstrate that your work meets the required revel 5 criteria.
