Problem
Imagine returning to nursing school and you are task with learning how to construct a care plan base on a scenario/case study given with specific questions to answer and complete. It would read as follows.
As Julie is performing her assessment rounds at the beginning of the shift, she notices as she assists Mrs. Jones to the restroom, this patient's ankles are very swollen, and she is significantly short of breath by the time they get her back to bed. Julie takes the patient's blood pressure and heart rate while obtaining a current oxygen saturation reading and makes notes for a call to the physician. As Julie recalls the parts of the SBAR, she writes each assessment finding, as follows: Situation-As she was walking Mrs. Jones to the restroom, she noticed that Mrs. Jones was short of breath after ambulating a short distance. Background-Mrs. Jones has been getting Lasix 20 mg IV every day since admission. Assessment-Mrs. Jones has 3+ edema to bilateral lower extremities and an oxygen saturation of 88% after ambulation, and she heard wheezing in Mrs. Jones lungs. Recommendation-does the physician want Mrs. Jones to have an extra dose of Lasix and an increase in her daily dose? The physician orders Lasix 40 mg IV now and increases her Lasix to 20 mg IV every 12 hours.
