Q. Termination and Follow-up in diabetes mellitus?

After plan is implemented, you have to assess the progress. Slowly start winding up the contact with the client and the family by providing them support, motivating the client to take care of oneself with the help of family when needed. Motivate the family to support patient and developing their coping strategies. In this way you will plan termination of contact.

Termination should not be sudden and it should be taken as a positive step by client, family and counsellor with no emotional involvement. But you will do the follow up of the client to ensure client is following the plan with no difficulties,  is  able to solve problem if any. Ensure patient is following the instructions of the doctor for medicine, insulin injection, diet, exercise, follow up with doctor, monitoring blood and urine sugar, checking blood pressure, taking care of skin, feet, eyes and if any complication visiting the doctor, family support and emotional stability of the client.

In this section you have learnt about steps of counselling process. It is a slow and continuous process. Maintain good relationship, provide privacy, good relaxing environment, be a good listener, ask simple questions, ask one question at a time, give time to client to think and answer, slowly, probe in detail, focus on  the main problem, find resources, motivate client, involve family, strengthen coping strategies, find alternative solutions, make a plan, implement it, monitor it, record all events. Slowly terminate the relationship with no emotional involvement and follow up the client.
