Original Contribution: National policy is trying to shift to paying for "quality of care" rather than quantity of care (or the fee-for-service model). In this week's discussion, discuss barriers to implementing a policy of focusing on "quality" rather than "quantity." What will be difficult in moving away from "fee-for-service" type care?

This shift would be huge in the health care system and take a lot of fine tuning. Insurance companies will have to redefine how they make money and they will make sure they are still going to be able to make as much or more as they were on the fee-for-service model. If that doesn't happen, they can slow down the change process dramatically.
In America, the more money you have the more power you have-- so insurance companies that see policy changes coming that affect their revenue put lobbyists in Congress and throughout politics to sway people another way-- they also give large campaigns contributions to candidates that will follow their agenda.
Another barrier to implementing these changes is doctors. They are so used to trying to squeeze in a lot of patients throughout the day to get more money as well. I think once it is implemented though, doctors will shift slowly to actually helping people stay healthy and being glad when they get paid more for keeping people well.
