Task:

Write a report (max. 4 pages) on an engineering failure of a consumer product. Present the failure analysis in the style of a technical report. Introduce the failure with supporting information. Recount the mode of failure and cause of failure for the event being analysed. Discuss at least TWO implications the failure has caused. These implications may include social, cultural, legal implications. Support the argument with other similar failure cases and factual data if applicable. The information sources should vary in the types of documents and be relevant, contemporary and authoritative. Inclusion of these references would reflect a comprehensive literature review. The referencing should be of the IEEE style.

Any included figures and graphs must have associated captions and references if applicable. The heading of the report should be "Failure Analysis: [your choice of failure]". The recommended parts to the document are of the following:

1. Introduction (of the failure event),
2. Background (Any related contextual information),
3. Failure (the failure),
4. Discussion (discussion of two implication the failure analysis has resulted)
5. Conclusions (summary and possible recommendation),
6. References (IEEE style).
