The majority of health care spending and deaths in the United States are related to expensive, debilitating chronic diseases such as obesity, diabetes, high blood pressure, heart disease, and cancer, which can be reduced or modified through behavioral interventions. The 2013 National Health Expenditure Accounts report by the Centers for Medicare & Medicaid Services (CMS) showed a 3.6% increase in health care spending in the United States, which stood at $2.9 trillion, or $9,255 per person per year.
By 2023, health expenditures financed by the federal, state, and local governments are projected to account for 48% of national health spending. Racial, ethnic, and sexual minority populations are disproportionately affected by certain chronic conditions owing to health barriers caused by lack of education and socioeconomic, geographic, or other sociocultural factors.
In 2005, 113 million people were living with at least one chronic disease, a number that is projected to grow to 157 million by 2020, affecting especially those below the federal poverty line. Given the aging population and associated disease-specific risk factors and social determinants, health care spending is expected to rise from $1.3 trillion in 2003 to $4.7 trillion in 2023.
