Patient Problem List for the Clinical Evaluation of a Patient in the EHR

Write a two page paper (excluding title and reference pages), and formatted according to APA style, that includes the rationale of a problem list and give at least two reasons why most clinicians use a problem list. Include examples of information that is included on a problem list and examples of things not included on a problem list. Explain why it is difficult to maintain the problem list and how the electronic health record can be utilized to maintain an up to date problem list.

Utilize a minimum of two scholarly sources (excluding the course text) that were published within the last five years, and are documented in APA style.

Paper must include at least two reasons why most clinicians use a problem list and the clinical information recorded in the chart to track one or more problem conditions.

Include regulatory requirements that require the maintenance of a problem list and their requirements for accreditation.
