You and a group of former classmates are planning a reunion weekend after 5 years of graduating from your university. You all now live in different countries. You have decided to meet in New York in 5 weeks' time. Each one of you is responsible for making your own travel and accommodation arrangements. While you have not made any specific plans of activities for the weekend, you have mandated a member of the team, John, to make enquiries and present them when you all meet. You plan to meet at your old university student bar at 2 p.m. on a Friday to begin the reunion weekend. In this Exercise, you will conduct an FMEA analysis based on the information presented, to identify things that could go wrong with the reunion plans.

Produce an FMEA analysis rate and calculate the risk priority number (RPN) to identify improvement actions. Then discuss how powerful an in-depth FMEA mode of thinking can be in an organisational process and the important benefits that such preventative approaches can deliver to any organisation.
