Behavior modification for demophobia

The best program of behavior modification for Demophobia for me was one-on-one sessions with a therapist who has experienced in treating phobias.  Many times the process starts off with an evaluation which you describe the fear to the therapist and then try to remember what may have contributed to it.   Afterwards, the therapist would review the evaluation with you and then propose a treatment plan.  This is called the feedback process.  The next step would be developing a fear hierarchy in which the therapist creates a list of scenarios that involves your fear while each scenario described is far more intense than the prior one.  My therapist informed me that this approach will involve gradual desensitization and exposure to Demophobia.  While attending these sessions, I learned how to tolerate the anxiety that is triggered by the exposure with the help of techniques that are designed to help you relax.  This process is known as exposure therapy.  The final step would be what is known as building.  This is the process where I have become comfortable with the relationship that the therapist and I have established and now I am ready to move forward with discussing very difficult situations.  It is very important to create a bond with a therapist who has your best interest at heart because I had been through several therapists who did not support or show any interest in helping me overcome my condition.

Below, I have listed the question that I need a response for and please include your references and sources..Thanks.

Describe the procedures with which you will implement your behavior modification program; for example, shaping, behavior chaining, differential reinforcement, etc. In addition, discuss the issues of selecting appropriate and effective reinforcement and/or schedules of reinforcement (if applicable).
